At the "main menu", press and hold R2, and then press and hold R1.
Release R2, then release R1.
Press and hold L2, and then press and hold L1.
Release L2, then release L1.
Press Circle, Square.
NOTE: If you entered the code correctly, you'll be taken to the level-select screen.
